Hospitals & Health Systems
We work with hospital discharge planners and social workers to place patients who need a safe place to recover before or instead of returning to the streets.
Social Service Agencies
Case managers, outreach workers, and social service providers refer clients to Haven when safe, stable housing is the critical missing piece in their care plan.
Treatment Providers
Recovery housing is a proven complement to treatment. We partner with treatment programs to provide the stable housing foundation their clients need to sustain recovery.

Making a Referral
How the referral process works
We've designed our referral process to be straightforward for busy care teams. Here's what to expect.

Submit referral form or call

Intake assessment
Our team will contact the person (or their representative) for a brief intake conversation to assess fit and identify the right program match.

Placement & transition
We work to place the person quickly and coordinate with your team to ensure a smooth transition, including any documentation or follow-up communication needed.

Ongoing communication
We maintain open communication with referring organizations throughout the person's stay at Haven, including updates on progress and next steps.
